3 tips from a doctor to save a man from premature ejaculation

The definition of "premature ejaculation" differs greatly between men and women. The Taiwan Society of Male Medicine released a study this year on "Awareness of Premature Ejaculation in Men", which found that 47% of men consider one minute to be premature, while 37% of women consider five minutes to be premature. According to urologists, the median time of delayed ejaculation of the penis in the vagina is 5.4 minutes, with an average of three to seven minutes being considered normal, but anything less than three minutes or even one minute should be considered premature ejaculation.

Dr. Zhuo Yuqing, attending urologist at the General Hospital of the Third Army, recently shared at the "Health LIFE Live" that the global prevalence of male sexual function problems include premature ejaculation 12.4-30.5%, difficulty in erection 12.9-28.1%, loss of sexual interest 12.5-28.0%, inability to reach orgasm 9.1-21.2%, unpleasant intercourse 7.7-17.4%, painful intercourse 29.-12.0%. 29.-12.0%.

Although the prevalence rate is high, the consultation rate is only 7.9 percent, which is quite low. The reasons for reluctance to consult a doctor include shyness to seek medical attention, low self-confidence and avoidance of sexual activity, as well as depression due to prolonged premature ejaculation.

According to Cheuk, the average median time of delayed ejaculation in the vagina can be objectively assessed as premature ejaculation, with a median of about 5.4 minutes. According to the Medical Association, premature ejaculation is defined as meeting three criteria, including timing, lack of control and psychological shadowing. Premature ejaculation may be "primary" if it is less than 1 minute and "secondary" if it is less than 3 minutes, while uncontrollable ejaculation or avoidance due to psychological shadows are also criteria.

The prevalence of "primary premature ejaculation" is less than 1 minute, which has been the case since the beginning of sexual activity, and is related to genetic and neurological factors, with a relatively low prevalence of 3.2%. The prevalence of "recurrent premature ejaculation" is less than 3 minutes, which is usually good but occasionally less than 3 minutes.

The most common variation of premature ejaculation is that it may be one minute yesterday, 10 minutes today, or six minutes the day after tomorrow, all of which are normal variations.

According to Cheuk, there are three reasons why premature ejaculation occurs: firstly, "psychological factors", such as a bad relationship with a partner, previous bad experiences, being hurt or ridiculed, etc.; secondly, "health factors", including obesity, inflammation of the prostate gland and thyroid disease, are all causes of premature ejaculation. Serotonin is a key transmitter in the brain, and abnormal levels can also affect premature ejaculation.

Premature ejaculation can be treated conservatively or with medication. According to Cheuk Yuk Hing, conservative treatment includes behavioural and psychological therapies. The three behavioural therapies are: "Stop-and-stimulate", where the penis is stimulated to the point of ejaculation, then stopped and stimulated again when the arousal level decreases, repeated three to four times. The "Stimulation and Sensitivity Reduction Squeeze" method involves squeezing the "nub" at the top of the penis when you are close to ejaculation, creating pain and reducing arousal, and then repeating the process. "After 12 weeks of treatment, the Kegel Exercise showed a 60% improvement.

In terms of psychological treatment, Cheuk said that it is important to understand the inner workings of a man's mind, such as arguments with his partner over premature ejaculation, and to reconcile and encourage each other, as well as to remove any anxiety about sexual performance. If behavioural therapy does not work, medication may be required. However, it is important to be aware of the side effects. Internal medication may cause headaches and diarrhoea, while external medication may cause numbness in the sexual organ and a burning sensation in the woman.
