The Science Behind Silver Wound Dressings: How They Promote Healing

Topical wound treatment products made from ionic silver are called silver dressings. These products have antimicrobial or antibacterial properties and deliver a consistent quantity of silver to the wound. The amount of silver that is activated from the dressing to the surface of the wound depends on the amount of exudate and bacteria that are present.

Silver dressings may be found as barrier layers, hydrocolloids, foam dressings, and dressings made of charcoal cloth. Silver foam dressings may be used in conjunction with topical and adjuvant therapy to reduce the bacterial load, control exudate, and improve the look of the wound's granulation tissue.

What characteristics of silver dressings should you look for?

There are four primary classifications for silver foam dressings. These silver treatments have the following effects: 

  • Discharging silver for antibacterial activity into the wound bed; 
  • Allowing the silver wound dressing fabric to absorb wound exudate, at which point the silver begins to function as a bactericidal agent (into the dressing but not necessarily the wound);
  • Releasing silver into the wound bed for antibacterial action after absorbing wound exudate and/or 
  • Letting go of the silver and sulfadiazine moiety by releasing silver sulfadiazine into the wound, which cleaves in the presence of wound exudate. 

Consequently, the doctor should consider the silver product's effect on the specific wound before selecting a silver dressing and making necessary adjustments. The ways that different silver compounds release the bactericidal silver ions vary, as does the pace at which they do so. Slow-release silver wound dressings release minuscule amounts of silver, rapidly bound up by the chloride in the wound exudate. 

How much and rapidly silver diffuses from a dressing into the wound surface depends on the overall quantity of silver in the dressing and its crystal structure. The chemically active surface area of silver will be bigger when it splits among smaller crystals than it would be when it divides among fewer larger crystals. Whether more concentration, total quantity, or both of silver in a dressing would lead to more biologically active silver in the dressing and on the wound surface is still up for discussion and controversy. 

 On one side of the debate, a dressing containing a significant quantity of silver may overcome the existing wound environment when excess chloride is present. These proponents contend that the residual silver ions will have an antibacterial impact on the wound environment as the chloride binds to the silver and turns it inactive (AgCl). Others have claimed that a dressing's effectiveness is determined by the quantity of soluble silver readily accessible in the dressing. 

Certain manufacturers of silver products will take great pride in the amount of silver in their dressings. Nevertheless, it hasn't been shown that a silver dressingcontaining more silver always produces superior clinical results.
